Premium Only Plan Implementation Checklist
☐ Complete the Request for Services Worksheet and return to GDI with annual fee.
☐ If you have an HSA (Health Savings Account), please notify GDI so we may include the applicable IRS language.
☐ Add “gdynamic.com” as an accepted domain to avoid GDI emails being caught in SPAM filters.
☐ Notify your payroll administrator of your implementation of this pretax plan.
☐ Upon receipt of Request for Services and payment, you will receive the following from GDI:
|☐ Salary Reduction Agreement: All employees who want to participate in the plan are required to complete an Agreement. GDI does not need copies; however, they should be kept on file with your organization.
☐ Administrative Services Agreement: This is the Agreement between your organization and GDI to administer your plan. You will receive two (2) copies, please sign both and keep one for your records and return the other the GDI.
☐ Flexible Benefit Plan Document: This document contains your responsibilities as a Plan Sponsor. This document should be signed and kept on file as documentation you have adopted this plan.
☐ Summary Plan Description: All benefit-eligible employees should receive a copy of the Summary Plan Description.
☐ Nondiscrimination Testing Worksheet: The IRS requires pretax benefit plans be tested each year to ensure compliance with nondiscrimination rules. GDI provides this testing each year as part of our services to you. To test your plan, please complete the nondiscrimination test worksheet and return it to GDI. We will have your results within two (2) weeks of receipt. You will receive this worksheet each year approximately ten (10) business days after your plan renewal date.
