When will I be notified if a receipt is required for a Benny™ card transaction?GDI will mail you a letter notifying you that a receipt is required. Once you receive the letter, simply copy your original receipt and return it to GDI with the letter.I received a letter but lost the receipts, what shall I do?If you are unable to obtain an itemized receipt or statement from the provider or vendor, you may submit a paper claim with documentation for other eligible expenses that you paid for out of pocket. The amount of the documentation must equal or exceed the amount requested in the letter.Another option is to reimburse the plan for the transaction. Simply submit a check or money order in the amount of the transaction (along with a copy of the letter you received regarding the transaction) to GDI. If we do not receive the receipts requested or repayment within 30 days, your card will be temporarily deactivated. How do I re-activate my card?Either submit the documentation requested, submit a paper claim with documentation for eligible expenses paid for out of pocket or send a check to GDI for the total amount requested.Does the card require a participant credit check?No, the card is pre-loaded with the participant’s election.Can my legal spouse or tax-dependent children use my card even though it is in my name?Yes.What happens if I lose my card?Call GDI at 800-626-3539. There is a $10 fee for a new card which will be deducted from your balance.Do I need my PIN number at the point of sale?No, the number assigned to the participant is to manage the account online, not for debit transactions. The card can only be used for credit transactions. |
