Submitting FSA Reimbursement Requests

GDI makes it easy to submit reimbursement requests for flexible spending account
participants by using theParticipant Login portal or follow the steps below:

1. Complete one of the following Reimbursement Request Forms:
2. Attach itemized receipts

3. Submit the form and documentation to GDI via any of the following methods:

  • Mail: 411 US Route One, Falmouth, ME 04105
